About the role:
The Clinical Care Coordinator works closely with the Clinical Care Manager providing leadership in the physical and psychosocial environment at IERA VIC that attains best practice and high quality clinical services and care outcomes for consumers.

Understanding and integrating the values and philosophy of Consumer Directed Care into work practices, behaviours and change management will be vital to be successful in this role, as will be confidence in Clinical Care capabilities.

Promoting a culture of Consumer Centred Care, co-design of systems with the consumer and continuous improvement are vital to this role.
Duties and Responsibilities:
Being involved in improvement processes striving for care best practice;
Attaining accurate documentation to support the Aged Care Funding Instrument including meeting the scrutiny of ACFI Auditing, balanced with Consumer Directed Care.
Undertaking care plan evaluations and overviews leading to the identification of opportunities to apply best practice to attain consumer outcome improvements;
Contributing to identifying an appropriate blend of workforce to meet needs goals and preferences of our consumers;
Providing support to the performance critical workforce in the form of frameworks, policies and procedures that help meet requirements that emerge from a growing understanding of consumer’s goals, needs and preferences. 
Active participation in identifying and solving problems relating to areas of responsibility including:
Prompt, effective resolution of issues related to the co design of Health and Personal Care for consumers and other related services;
Managing and mitigation of risks relating to consumer safety and quality of care and services;
Participating in the development and implementation of consumer centred lifestyle programs that optimises consumer life fulfilment.
Co-leadership of the quality and safety of clinical care services within IERA VIC.
Contributing to workforce engagement and education to improve consumer clinical outcomes.
Assisting in reviewing workforce performance, competencies and behaviour to develop and implement relevant learning programs.
Skills and Experience Required
To be successful in this role you will need to have;
Demonstrated excellence in clinical care in an aged care setting of no less than 3 years, with examples of best practice implementation.
Previous leadership experience of no less than 2 years with the ability to demonstrate continuous improvement.
Experience in managing/mentoring care staff and success in managing change
Current registration as a Registered Nurse Division 1
Strong knowledge of aged care standards that apply from 1 July 2019 and regulatory compliance related to Clinical Care and Services.
Demonstrated excellence in communications skills at all levels of the organisation and with diverse consumers.  It would be an advantage to be bilingually proficient in Vietnamese and English.
Proven success in maintaining and managing safe and quality care and service delivery.
Demonstrated experience in understanding what is happening at the point of care and how this can be improved, with examples that illustrate this.
Strong computer skills with Microsoft Office Package and a demonstrated ability to optimise technology solutions to clinical care and services, including consumer aids.
Proven experience in coaching and mentoring staff, supporting staff to manage change, as well as practical experience in caring for consumers with Dementia and other clinical needs.

Ensure Assistants are part of care plan evaluations with consumers and leading them in identifying opportunities to apply best practice to attain consumer health and wellbeing improvements;
Contributing to identifying an appropriate blend of leisure and lifestyle workforce, including volunteers, to meet the leisure and lifestyle needs, goals and preferences of our consumers;
Developing a strong network of community contacts to help in fulfilling consumer needs, goals and preferences;

Experience in managing/mentoring staff and success in managing change. Certificate IV in Leisure and Lifestyle.
Prior experience in developing and recommending Lifestyle and Leisure budgets, policies and procedures and workforce performance appraisal and management.
Strong knowledge of aged care standards that apply from 1 July 2019 and consumer experience evaluation of programs.
Demonstrated excellence in communications skills at all levels of the organisation and with diverse consumers.  It would be an advantage to be bilingually proficient in Vietnamese and English.
Demonstrated experience in understanding what is happening at the point of care and how leisure and lifestyle components including exercise can be incorporated in to daily living, with examples that illustrate this.
