Care Partner – Support at Home (SAH) Program
Full-time | Melbourne’s Western Suburbs, Victoria
Indochinese Elderly Refugees Association (IERA) is seeking a Care Partner to join our Support at Home Program.
The Care Partner provides a valued and key role in delivering high quality, professional and integrated support services to elderly Support at Home participants in the community. The main responsibilities include developing tailored care plans, managing budgets, identifying and mitigating risks, and coordinating care services to enhance our participant’s quality of life and maintain their independence.
Key Responsibilities
- Serve as the first point of contact for SAH enquiries and referrals.
- Conduct intake screening, eligibility checks, and initial client assessments.
- Explain program services, fee structures, rights, and responsibilities.
- Use support plan recommendations to develop care plan goals
- Setting up and maintaining budgets and accountability reports
- Refer and liaise relevant health professionals and services to support coordination of care for the participant
- Meet individual and team KPIs and targets to meet funding requirements
- Schedule and roster shifts for care workers
- Submit invoices and claims on behalf of clients to Services Australia
- Liaise with the Finance Officer to ensure that SAH operates as an efficient cost center.
What You Bring
- Certificate IV, Diploma, or tertiary qualification in Community Services or a related field
- Sound knowledge of the Support at Home (SAH) and CHSP programs, Aged Care Quality Standards, and the new Aged Care Act (from 1 November 2025)
- Fluency in English and Vietnamese (essential)
- Minimum two years’ experience in SAH package management or a similar aged care role (preferred)
- Experience supporting older people from CALD backgrounds
- Experience in client engagement and budget monitoring
- Knowledge of Home Care Packages and service delivery models (preferred)
- Marketing and influencing skills would be highly regarded

IERA Requirements
- Current Victorian Drivers License
- Valid Working with Children Check
- Travel between sites will be required
- First Aid Certificate
- Satisfactory Police Check
