What is a Social Support Group?

Social Support Groups are part of the Federal Government’s Commonwealth Home Support Program (CHSP) and the State Government’s Home and Community Care Program for Younger People (HACC-PYP). These programs are designed to assist eligible individuals in maintaining their independence, improving their functional capacity, and enhancing their participation in social activities, both at home and in the community. Client contribution fees apply.

Our Social Support Program encourages active participation in social activities with others, whether through centre-based events or community outings. We use the ‘Wellness & Reablement’ approach to empower you to engage in activities that interest you, all while supporting your independence and keeping you connected to the community.

  • Ongoing encouragement to maintain or improve your social and physical health.

Support Care: How We Assist You

To ensure convenience and accessibility, our Social Support Group offers transportation via a small bus driven by our trained staff. This service is available for pick-up and drop-off, ensuring you can attend activities comfortably. However, individuals who are able are encouraged to make their own travel arrangements to and from the group.

All staff and volunteers are trained and experienced in supporting older adults and their carers, ensuring you receive compassionate care and assistance throughout your participation.

  • Information Sessions & Monthly Outings
  • Events & Celebrations

Using a person-cantered approach, we focus on the skills needed for independent living. We continuously use feedback from participants to tailor activities that are both enjoyable and beneficial.

How to Join Our Social Support Group Program

Accessing government-subsidised home and community support services starts with an assessment. Please contact the appropriate assessment service to get started:

  • My Aged Care – For individuals aged 65 years and over (or 50+ for Aboriginal or Torres Strait Islander peoples).
    • Phone: 1800 200 422 or visit My Aged Care to make a referral.
  • HACC PYP Assessment Officer – For individuals under 65 years and not eligible for NDIS (or under 50 years for Aboriginal or Torres Strait Islander peoples).
